This article explains the different ways to add a new form template to your Forms & Flows library, depending on the type of form you need.
Overview: Two Types of Templates
You can create two main types of form templates in Forms & Flows:
- Standard eForms: Created directly by you within the platform. Best for simple to complex digital forms.
- Formatted/Mapped Forms (PDFs): Created by our team from your provided PDF. Best for forms that must match a specific pre-existing paper layout.
The process is different for each, as outlined below.
Create a Standard eForm Template
This is the quickest way to create a reusable digital form for things like permission slips, surveys, or health updates.
- Navigate to the Forms & Flows section in your dashboard.
- Click the “+ Create” button.
- Use the form builder to add your questions, fields, and logic.
- Once you are satisfied with your form, click “Save as Template” (usually found in the "Save" or options menu).
- Give your template a clear and descriptive name (e.g., "Annual Field Trip Permission Slip").
- Click Save. Your new template is now available in your personal template library for you and your team to use anytime.
Best for: New digital forms, permission slips, surveys, questionnaires, and health checks.
Formatted/Mapped PDF Form Template
Do you have a pre-existing form, like an official district PDF, that needs to be digitized exactly? Our team can map each field from your PDF into a fillable Forms & Flows eForm.
How to request one:
- Prepare your PDF: Have the final, high-quality version of the form you want digitized ready.
- Contact Support: Please send your PDF as an attachment to our support team.
- Provide Context: Include a brief note explaining the purpose of the form and any urgent deadlines.
What happens next?
Our development team will create a meticulously mapped template from your PDF. This process ensures the digital version matches your original layout exactly.
- Please note: Forms with a very high number of fields are more complex to build and will require additional time to complete.
Once finished, the new mapped template will be added to your library.
Best for: Official district forms, IEP forms, medical forms, and any document that must match a specific paper layout.
