Forms Hub - Parent and Student View
The hub has been simplified, with the functions used the most each having its own button. You will no longer need to use menus to find Self Service forms. The purple number indicators on the first screenshot relate to the explanations in the tables below.
Form Options
Description | What it Looked like Before | What it Looks like Now |
1. Form Requests:New forms for completion assigned by the school |
Click the first (grey) button labeled Form Requests (The number indicates the number of outstanding forms): Each form will have its own card, click Start Form to complete the form. |

NEW! 2. In-Progress Forms:When you have saved a form for later, they will show here until submitted |
Click the second (blue) button labeled In-Progress (The number indicates the number of in-progress forms, if there is no number, there are no in-progress forms): A list of in-progress forms will be displayed, click Continue to access the form. |

3. Completed Forms:A list of every form you have submitted |
Click the third (green) button labeled Completed Forms: A list of completed forms will be displayed, click View Form to access the form. |

4. Form Center:Start and submit a Self Service form |
Click the fourth (yellow) button labeled Form Center (The NEW badge indicates a new self service form is available): A list of Self Service forms will be displayed, click Start Form to access the form. |

5. My Students:View your student’s forms and record. |
Click the fifth (purple) button labeled My Students: A card for each of your students will be displayed. There is an option to View Forms or View Record for each student. |
