Adding an action plan while creating a New Medical Condition
1. Click the profile name or photo.
2. In the Profile Details page, go to the Medical Condition section and click the Edit button.
3. Tick Yes for Asthma and click Save and Continue.
4. On the next page, scroll down until you see the Upload an attachment (PDF file or Picture) section. Click Choose File, and find the action plan in your computer or smartphone.
5. If the action plan needs to be updated after a specific date,
6. Fill out the form completely as necessary and tick the "These are my approved care instructions" box. Then, click on the Update button.
7. You will be taken to the Consent section. As a change has been made to the Care Profile, you will need to re-consent, confirm that the profile has been changed and that the medical information contained within is up-to-date.
Adding an action plan to an Existing Medical Condition
1. Click on the name of the profile.
2. At the top of the page, click on Full View and select Asthma from the drop-down.
3. At the bottom of the page, select Choose File and find the asthma plan in your computer or smartphone. If the action plan needs to be updated after a specific date, please choose a date for the Review Date/Expiry date for Action Plan.
4. If all details are updated, tick These are my approved care instructions* and click Update to complete the process.
5. You will be taken to the Consent section. As a change has been made to the Care Profile, you will need to re-consent, confirm that the profile has been changed, and that the medical information contained within is up-to-date.
